- Enrollment, transaction, and payment-status information. Payment-card details are processed by JotForm and its connected payment provider; Enov does not intend to store full payment-card numbers on this website.

We use information to provide requested services, review questionnaires, communicate with you, schedule appointments, process enrollment, operate the client portal, maintain records, prevent misuse, improve the website, and comply with applicable obligations.
We do not use information submitted for one service to guarantee a credit result, funding approval, or business outcome.

Sensitive portal records and uploaded documents are intended to be access-controlled, and private documents are not intended to use public storage links. No internet or storage system can be guaranteed completely secure.
